Its total speculation on my part, but there are many cases of people deliberatly flouting safety rules for many reasons. Perhaps to do the their job more easlily, or more quickly, or they are too lazy to do the job properly.
(And I have to be honest and admit I have done all of these myself in the past.)
So if this worker had deliberalty removing switch tie-rod bolts, for any of the any of the above reasons then he would be personally responsible.
