Usually these are buses off route due to a diversion or a driver just taking a wrong turning or maybe even a private charter. Even if it's not the normal route, I cannot understand a driver of a high vehicle (double decker bus or high sided lorry) paying so little attention that he (or she) fails to notice something like a bridge.
That said, bridge strikes are so common (I believe it's more than one a day nationwide), and presumably expensive, I don't understand why Network Rail don't put more investment into electronic warning devices which give early warning of low bridges. Ther's one near us and it seemas to work well.
