It was not the drivers fault, when he pulled the points you could see that he looked twice to see if they were fitting up.
The train seemed to be going slowly, and quite a few trucks went through the points ok so it seems there was an axle fault causing a wheel to ride up onto the rail head or the point blades were forced open by the passage of the tain.
The trucks all turned over due to the coulpings holding.
The brakes would have applied by the driver, or a pipe coupling parting between a derailed wagon.
In my view the driver was not to blame.
