yeah thats bilge.....i bet even the driver has said so...

as for responsibility, I imagine that the Prosecution will be asserting that the missing bits were wilfully and deliberatley left off....this is the only case I can think of where simply the guy with the spanner would be solely responsible. Doesnt happen? Read Adrian Vaughans book on Train Crashes for at least two examples of rail staff delberatley causing an accident! Proving it is very difficult though...
